When Years of Physical Therapy Didn’t Solve Shoulder Pain: How an Integrative Approach Made a Difference

When Years of Physical Therapy Didn’t Solve Shoulder Pain: How an Integrative Approach Made a Difference

One of the most rewarding parts of being a physical therapist is seeing patients regain the ability to do things they once thought were impossible. Sometimes, it’s not about finding a “miracle” treatment—it’s about finding the right combination of treatments based on the patient’s individual needs.

Recently, I ran into a former patient in the community. As we caught up, they smiled and said, “My shoulder is still doing great.”

Hearing that months after treatment is one of the best compliments a physical therapist can receive.

A Shoulder That Hadn’t Improved

This patient had been living with shoulder pain and limited range of motion for quite some time. They had already completed multiple rounds of physical therapy over the years. While they experienced temporary improvements, the pain and stiffness always seemed to return.

Everyday activities had become frustrating:

  • Reaching overhead

  • Getting dressed

  • Lifting objects

  • Sleeping comfortably

  • Performing household tasks

Like many people with persistent shoulder pain, they had started believing this might simply be their “new normal.”

Looking Beyond the Shoulder

At Precision Point PT, treatment always begins with a comprehensive evaluation.

Instead of focusing only on where the pain was located, I wanted to understand why the shoulder wasn’t moving normally.

During the evaluation, I assessed:

  • Shoulder mobility

  • Joint mechanics

  • Muscle strength

  • Movement patterns

  • Posture

  • Scapular control

  • Soft tissue restrictions

  • Functional limitations

Pain is often only one piece of the puzzle. Muscles can become overactive, weak, or poorly coordinated, causing the body to compensate in ways that limit movement and prolong discomfort.

Why Integrative Dry Needling?

After the evaluation, I determined that Integrative Dry Needling (IDN) was an appropriate part of the treatment plan.

Unlike using dry needling as a stand-alone treatment, IDN combines needling with a full physical therapy assessment and rehabilitation plan. The goal is to improve movement, reduce pain, and help the body function more efficiently.

For this patient, dry needling was used to address muscle tension and improve mobility, while other physical therapy techniques reinforced healthier movement patterns.

Immediate Improvements

Following treatment, the patient experienced a significant improvement in shoulder range of motion and reported noticeably less pain.

While every patient responds differently, improvements like these can occur when muscle guarding decreases and normal movement is restored.

The treatment didn’t end with the needles.

We also discussed movement strategies and exercises to help maintain those improvements and support long-term recovery.

The Best Part Came Months Later

Several months after our visit, I unexpectedly ran into this patient in the community.

Naturally, I asked how the shoulder was feeling.

Their response was simple:

“My shoulder is still doing great.”

There were no reports of the pain returning, and they continued to enjoy the improved motion they had gained after treatment.

As a physical therapist, there’s nothing more rewarding than hearing that the progress lasted beyond the clinic.

Every Patient Is Different

It’s important to remember that no two patients are alike.

Not everyone experiences dramatic improvement after a single treatment, and Integrative Dry Needling is not appropriate for every condition. Recovery depends on many factors, including the diagnosis, duration of symptoms, overall health, activity level, and commitment to the treatment plan.

That’s why every patient at Precision Point PT receives an individualized evaluation before any treatment recommendations are made.
